Being disturbed: the impact of severe personality disorder on professional carers

Abstract

Ogden Nash’s quip implies a perfectly neutral attitude on the part of a professional to the affairs of the people he deals with. Perhaps it is a familiar view of professional people. We need our accountants, lawyers and so on to give us an objective view of our affairs. Maybe that should be what is needed from the caring professions. However, I have argued that within psychiatry carers are particularly disconcerted by the work they do (Hinshelwood, 1999; 2004). In the present paper, I wish to look in some closer detail at these interpersonal processes that occur between carers and those they care for, with particular emphasis on personality disordered people.
This Chapter will take a critical perspective, questioning and problematising the standard approach to care provision based on the consumer model as developed since about 1990 in the NHS. The critique will involve a perspective based on a relational model, in which ‘users’ will be recognised as in relation with their carers, rather than merely objects of care; and indeed ‘carers’ are, equally, in a relationship with their users. This article testifies about the work of short intervention realized in a camp of evacuees built and managed by an Association of Civil Protection’s Volunteers, two weeks later the earthquake which damaged Emilia Romagna on the 20th of May 2012.

The group activities in the Diagnosis and Treatment of Psychiatric Services

Abstract

In all the western countries has established the model of community psychiatry. In this model, the aims of the wards for acute pathologies (Psychiatric Emergency Ser-vices) are slowly passed from the historical ones, of the control of agitation, anger and despair, with the limitation of the most disturbing expressions on the social level, taking no notice of diagnosis and evaluation of the causes of the crisis, to the most modern ones, which measure the quality and success of a inpatient acute psychiatric ward not on the number of patients seen and stabilized, but instead entrusted with success in long-term therapy project.
In this perspective, the group activity within the Italian Diagnosis and Treatment Psychiatric Services (SPDC, the inpatient acute psychiatric ward of the NHS) takes a significant value, both for the management of the emotional atmosphere inside the ward, that for the development or the reinforcement of those social cognition skills that will be, at discharge, decisive for the recovery of adequate cognitive skills.

Why psychoanalysis in times of poverty?

Abstract

The psychoanalytical group is characterized by a self-representative work and what happens within it, is considered as an useful material for putting in meaning the same group. Thoughts, affections, somatic expressions, sounds, noises, etc. tend to form a multi-dimensional tissue called with a good term, by Francesco Corrao (1979), <<self-interpreting context>>.
By this way the group can be operationally defined by the setting which establishes its limits but it is at the same time, in a dynamic relation with the reality’s levels in which it is immerged.
I try of placing with this article, some founding moments of Bion’s model which came from his experiences, in the way of highlighting, referring to the case of groups applied within institutional contexts, the importance since the origins, of a field’s perspective in which the clinic practice will never be a technical operation extracted from the institution but an integral part of it.
In a “ill” institution it is highly improbable that “healthy” groups can born and develop. Group psychotherapy with alcoholic patients: some theoretical and clinical issues

With this article we hope to illustrate how changing theoretical views within the psychoanalytic understanding of the mind and psychopathology of the alcoholic patient have lead to a shift in our clinical approach of these difficult patients. Starting from a more object relational approach we have integrated important contributions from self psychology and recently also from the work of Monjauze on the alcoholic part of the personality. Furthermore, we are thinking of ways in which we could apply the work of René Kaës in our group psychotherapeutic work with this population. His description of the psychic formations in the three psychic spaces, and especially his concept of the phoric function within a group has caught our attention.
